health care is a service that permits
patients to receive quality health care in
the privacy and comfort of their homes.

Why home health care?
Home health care is a
cost-effective option for receiving health
care services. Returning to one’s home and
family may speed recovery and can improve
the quality of life for both patient and
family or caregiver.

What criteria must be met for
Medicare to approve services?
The following criteria is used to meet
Medicare requirements:

The patient
must be covered by Medicare Part A or
Medicare Part B.

The patient
must be homebound. This is defined as
normal inability to leave home and that
where leaving the home requires a
considerable or taxing effort.

The skilled
care must be medically necessary as
determined by the physician.

What types of health care can be
provided at home?
Many medical conditions
that previously required hospitalization can
safely be treated in the home. Monarch Home
Healthcare Agency offers these basic
services:

Skilled
nursing care, including skilled nursing
visits for Wound Care, Diabetic Care,
Orthopedic Care, Cardiovascular Care,
Pulmonary Care, Catheter Care and
patient and family teaching.

Rehabilitation therapies, including
physical therapy, occupational therapy
and speech therapy.

Medical
social work.
-
Certified
Nurse Assistants for activities of daily
living services.

Physician’s
house calls. Doctor’s can visit patient
in their home for any medical need.

Who pays for home health care?
Most health insurance
companies, Medicare, Medicaid, Medicare
HMOs, Worker’s compensation. Some insurance
providers do not cover all home health
services. Patient will be responsible for
the payment of services or balance that your
insurance did not cover. Monarch Home
Healthcare Agency staff will verify health
coverage for the patient before start of
care.

What if I have a problem at night
or on the weekend?
Monarch Home Healthcare
Agency has a registered nurse on call 24
hours a day, seven days a week for urgent
needs.

Do I need a physician’s orders for
home health care?
YES. All health care
provided in the home is under the direct
order and supervision of the patient’s
primary care physician. Some medical
supplies do not require a physician’s order
and are available for direct purchase by the
patient.
