Patients with one or more of the following conditions may be eligible for Home Health Care services:

• New surgical procedure.

• New or changed in your medical diagnosis.

• Difficulty walking, transferring, or performing regular daily activities.

• New or changed medication.

• Problems with talking or swallowing.

• New medical equipment such as oxygen, a walker, blood sugar meter, etc.

• New or changed caregiver.
